Kad Peduli Sihat / Health Care Card
The Government of Selangor in Malaysia has launched the Health Care Card Scheme under the Selangor State expenditure. This scheme was launched to help those with low-income in the state in order to handle the problem when it comes to the cost of medical expenses. Under this scheme, those who are eligible will receive a Health Care Card with a value of RM500 and RM200 per year for free. There are two categories for the Health Care Card, Category A and Category B.
For Category A, one family can get a health care card that applies to the main account holder and the family. So, what are the benefits of this card? Well, the card has a total value of RM500 per year and the card can be used to a total of RM50 for one treatment at a time. Most importantly, there are no restrictions to any kind of medical treatment for this card. It is applicable for the husband or wife and their children under the age of 21 years old. The household income must be from RM3,000 or less per month. Besides being Malaysia citizens, applicants must be Selangor-born or have lived in the state for at least 10 years.
For Category B, the card is for individuals aged from 21 years old and above. The card has a total value of RM200 per year and can be used to a total of RM50 for one treatment at a time. Similar to Category A, there are no restrictions to any kind of medical treatment for this card. Those eligible for Category B must be a Malaysia citizen aged 21 years old and above with a monthly income of RM1,500 or below. Applicants must also be Selangor-born or have lived in the state for at least 10 years.
For the moment, there is no longer online application that can be made. Those who are qualify can apply for the card by downloading the form online. This form must be filled out completely and sent to the nearest office of the Selangor State Assembly after 22 December 2016.
